Synopsis
The author reflects on issues facing health care professionals in assessing and managing older people with multimorbidity or those with two or more chronic medical conditions, particularly dementia, hearing loss, and vision impairment. Also cited are the risk of isolation, loneliness and disability in older persons with multimorbidity, the definition of successful aging, as well as the components of a person-centred approach in elder care.
